In themselves, these venous dilatations are physiological. Their presence is normal, hemorrhoids forming kinds of pads that line the inner wall of the anal canal with a precise function: to ensure the fine regulation of the sphincter and its good closure. Normally, therefore, hemorrhoids, in particular, control the elimination of digestive gases. And this “gas plant” is divided into two hemorrhoidal networks: external at the margin of the anus, and internal in the anal canal, located above.

A simple gravity … or a sharp pain

As a result of anatomical deterioration or circulatory disorders, phenomena similar to varicose veins can develop in hemorrhoids. It is at this point that the signs of what is called hemorrhoidal disease appear. And it is also at this moment that the patient “discovers” that he – or she – “has hemorrhoids”.

This is very common since one in two people has had or will have hemorrhoids at least once in his life! Current, the phenomenon is multifaceted. It results in a gene, a gravity, itching or a burning sensation, or even severe pain in the anal area. The stools may contain bright red blood – which will stain the toilet paper or splash the bowl widely … Sometimes also when wiping, the patient discovers a small hard and painful ball near the anus.

Another very important point: the pain is not at all correlated with the volume of the dilatation. Large hemorrhoids can remain painless indefinitely, while very small dilations can lead to very painful pain.

Consultation with the doctor is needed The hemorrhoidal crisis may be common, it is not much talk. By modesty, as if it were a shameful disease. Too often and always, for this reason, the patient takes shelter behind a pretext – “hemorrhoids, it does not matter! – not to consult the doctor.

But it is imperative to consult. Because the symptoms, if they translate most often a haemorrhoidal crisis, hide sometimes another disease like an anal fissure, whose treatment is different, even a pathology otherwise serious as a cancer of the colon … Only the doctor -generalist, gastrointestinal enterologist or proctologist – will be able to exclude these possibilities by the interrogation and a careful examination, then to prescribe a suitable treatment … and refunded by the social security.

From “rounds of leather” to heredity … The causes of hemorrhoidal disease are poorly known, but there are some well-established certainties. On the one hand, there are indeed predisposed families. And on the other hand, several contributing factors are known:

  • Constipation or diarrhea, both intense or prolonged. It is the same for certain episodes, however natural of the genital life, such as pregnancy and childbirth, even the rules;
  • Diet also has an influence: many hemorrhoidal crises occur in lovers of good food, the day after a large meal that included a game or meat meats, or spicy dishes, all washed down with a good heady wine possibly followed by tea or coffee and strong alcohols in “coffee-grinder”. This is not surprising: all the dietary factors favoring the occurrence of hemorrhoids are gathered in this menu!
  • Not to mention the long hours spent at the table, because the prolonged sitting position contributes to the occurrence of a crisis. In fact, it was precisely to avoid hemorrhoidal attacks that office workers – sitting for long hours – put on their seats “rounds of leather” made famous by Courteline.
  • Without being sedentary, as part of their job, some of us spend long hours sitting and are exposed. Drivers, taxi drivers, cyclists, bikers and riders are, particularly at risk.
  • Finally, the abuse of laxatives, suppositories or enemas may be involved.

Internal or external?

External hemorrhoid, which appears as a protuberance near the anus, causes itching and the emission of red blood during the stool. It also happens that it is the seat of inflammation. And if a clot forms there, it becomes hard and very painful. The good news is that the evolution of these external hemorrhoids is benign. The symptoms regress in a few days with medical treatment and good control of the hygiene of life.

Internal hemorrhoids, on the other hand, become more complicated than external hemorrhoids. They too can only result in itching or bleeding. But sometimes they descend along the anal canal and externalize, forming a very painful hemorrhoidal “package” in the anus. The phenomenon can be intermittent – for example when going to a bowel movement – or permanent. Finally, these internal hemorrhoids can be trapped by the sphincter. They then constitute a hemorrhoidal constriction: a surgical emergency in its own right.

A treatment prescribed by the doctor

Depending on the severity, the treatment will be medical or surgical. When the crisis comes down to a gene, a gravity, a painful tension, small bleeding – which is most often the case during the first attack of external hemorrhoids – the doctor will prescribe veinoprotectives and/or antispasmodics. Oral, in the form of suppositories or creams in local applications. They aim to restore the structure of the vessels. Against pain, an analgesic, antispasmodic and anti-inflammatory approach is also recommended, generally or in local applications. Finally, local anesthetics can be very effective against pain.

All these drugs can be combined in one and the same preparation. Which facilitates the catch. Finally, it may be useful to prescribe mild laxatives – that is to say, not irritating to the rectal mucosa – or paraffin oil to soften the stool.

After treatment of external hemorrhoids by thrombosis, it is possible to hasten to heal by incision. It causes immediate relief but does not protect against recurrence. The excision, which removes all the lesion and leaves no scar, is superior to it. In either case, the proctologist or the gastroenterologist obviously practices under local anesthesia …

Simple rules whatever it is, the treatment must absolutely be completed with a hygienic and dietary approach. The generalist will not fail to provide them, naturally. Against possible constipation, it will recommend you to drink in quantity – at least 1.5 liters per day, 2 liters if you can … – and to adopt a diet rich in fibers. You can also take a mucilage, paraffin oil or use the lubricating virtues of glycerin suppositories. Conversely, it can also treat diarrhea.

He will advise you to avoid sedentary and prolonged sitting stations, to move – long live exercise and walking -, avoid cycling, long trips by car or motorbike, riding … And then … goodbye spices and alcohol, game meats such as a game, coffee, and tea …

Locally, you will ensure strict hygiene of the area: gentle washing with neutral or surgras soaps after each saddle, followed by careful drying. In case of itching, sweet almond oil on cotton. Finally, the doctor may recommend cold sitz baths twice a day.

The hemorrhoidal disease is therefore very common, most of the time without gravity, very accessible to the treatment as to the improvement of the hygiene of life. However, do not take it lightly and do not go to self-medication.

Its management must absolutely go through a visit to the doctor. Specialist or generalist, you choose according to your feelings. But he alone is able to determine if the symptoms are not related to a more serious cause and to prescribe effective drugs that will really help you. Any fit of modesty that would detain you from consulting the doctor would be … without foundation!
